About the Job:
LPNs responsible for participating in the provision of professional nursing care to patients under the supervision of Registered Nurses, Providers, and Practice Manager.
What You’ll Do:
Under supervision of the RN and Provider(s), participate in the nursing process, in-person, by telephone (telephone triage) and via telehealth modalities in accordance with organizational standards, state Nurse Practice Act, and other professional standards: performing nursing assessments, interventions, and evaluating progress toward defined goals of care.
Participate in designing and implementing a plan of care with the interdisciplinary team including Registered Nurses, Physicians, Advance-Practice Nurses (APRN, CNM, CRNA’s), Physician Assistants, and other health professionals.
Participate in educating patients on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
Document participation in the nursing process in the medical record with accuracy and detail.
Administer prescribed medications, treatments, and vaccines; perform point-of-care testing; collect and handle specimens within scope of practice as determined by the Nurse Practice Act of the appropriate state.
Who You Are:
Licensed Practical Nurse with current NH or compact state license.
Certifications: Current BLS/CPR training.
Graduate from an accredited school of nursing required OR currently enrolled in an accredited program leading to licensure as a Registered Nurse and has current NH license as an LPN required.
