PRIMARY DUTIES AND RESPONSIBILITIES

· Carry out the nursing process in-person, by telephone (telephone triage) and via telehealth modalities in accordance with Professional Scope and Standards of Practice (ANA, AAACN, etc.), state Nurse Practice Act, and other professional standards: assessing and identifying patient needs, planning to achieve appropriate outcomes, intervening, and evaluating progress toward goals of care.

· Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Advance-Practice Nurses (APRN, CNM, CRNA’s), Physician Assistants, and other health professionals.

· Educate patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).

· Document the nursing process in the medical record with accuracy and detail.

· Perform pre-visit planning to prepare the practice team for scheduled patient encounters, obtaining needed documentation, identifying preventive care and chronic disease management needs, and other medical services due.

· Assist providers with examinations, diagnostic procedures, and treatments within scope of practice as determined by the Nurse Practice Act of the appropriate state.

· Administer prescribed medications, treatments, and vaccines; collect and handle specimens and perform point-of-care testing within scope of practice as determined by the Nurse Practice Act of the appropriate state.

· Delegate patient care tasks to clinical staff based on scope of practice, and administrative tasks to clerical staff based on scope of responsibility, as appropriate.

· Identify and respond appropriately to emergency codes in accordance with facility guidelines.

· Provide clinical leadership to practice staff, including mentoring and coaching newly hired staff.

· Adhere to HIPAA and other confidentiality laws and regulations.

· Adhere to safety and infection control guidelines and promotes practice adherence to these guidelines.

· Attend ongoing education programs, including annual competencies in telephone triage and clinical skills day, offered by the Director of Nursing and Nurse Educator, as assigned by the Practice Manager.

· Stay abreast of ongoing changes related to EPIC integration and developing /growing Solution Health integrated initiatives as needed.

· Maintain up-to-date professional licensure and certifications, as required.

· Perform other duties as assigned.
