Elliot Health System - Clinical Dietitian - Diabetes Management - Full Time

To provide nutrition care and dietary management to adult and pediatric clientele in the Elliot Center for Diabetes Management (ECDM) and Elliot
Center for Advanced Nutrition Therapy (ECANT).

Key Responsibilities

  • Coordinates and integrates clinical aspects of dietetics for the ECDM and ECANT to provide quality nutrition care to outpatient clientele.
  • Provides nutrition assessment, planning, implementation, and evaluation to assigned patients in ECDM or ECANT a) Identifies and designs interventions which take into account patient needs related to age, cultural background, religious affiliation, medical status, and lifestyle. b) Interfaces with other members of the healthcare team for improved patient outcomes. c) Meets departmental standards for timeliness and documentation
  • Provides nutrition education to outpatients and their families in a group or individual basis based on assessed needs.
  • Documents all medical nutrition therapy and teaching activities.
  • Provides nutrition education programs for community organizations, including but not limited to: support groups held at Elliot Center for advanced nutrtion therapy and Bariatric clinic.
  • Establishes and maintains effective working relationships with patients and family support, the general public, physicians, physicians’ office staff and Elliot Hospital personnel.
  • Participates in Total Quality Management, development of department policies and procedures and in yearly planning of support group agenda.
  • Exercises care in the operation and use of departmental equipment and reference materials. Performs routine cleaning and preventative maintenance to ensure continuing functioning of equipment. Maintains work area in a clean and orderly manner.
  • Participates in appropriate role in disasters, fire drills, fires and other emergency situations.

Education/Experience/Licensure

  • Education: BS Degree from an accredited college or university and a coordinated undergraduate program Registered Dietitian.
  • Experience: Minimum of two (2) years of experience in clinical dietetics preferred. Past experience with Bariatric population preferred.
  • Certification/Licensure: Registration with the American Dietetic Association, and RD registration with the State of New Hampshire. Must maintain registration with the Commission of Dietetic Registration of the American Dietetic Association. Certification in Adult Weight Management and/or Diabetes Education preferred.
  • Software/Hardware: Proficient in computer use. Some experience in Electronic Medicat Records system is preferred.
