About the Job:
Provides care to the adult, adult sized adolescent & geriatric patients and families in the inpatient and outpatient Cardiovascular Health population with needs ranging from basic to complex utilizing the nursing process. Creates a therapeutic and caring environment for patient and families. Practices in a manner consistent with the Elliot Hospital Interprofessional Practice model; the ANA’s Social Policy Statement, Scope and Standards of Practice, and the Nursing Code of Ethics; American Association of Cardiovascular and Pulmonary Rehab (AACVPR) and American College of Sports Medicine under the direction of the Cardiovascular Services Clinical Nurse Manager and the Cardiovascular Services Medical Director
What You’ll Do:
- Demonstrates knowledge and competency in patient centric care related to cardiovascular rehabilitation modalities.
- Demonstrates competency in the 10 areas of core competency put forth by AACVPR.
- Assesses client’s physical and emotional adaptation to cardiac disease and wellness & the client’s/family’s level of health literacy, appropriately adapts teaching modalities to assessment findings.
- Provides education that can help facilitate health promotion behaviors to cardiac rehabilitation clients.
- Teaches clients and their families incorporating evidence-based teaching theories into practice; incorporates teaching techniques for adult learners.
- Provides education on various aspects of cardiovascular health, including, but not limited to: cardiovascular A&P, medication, social habits, smoking cessation, exercise philosophy, & diet.
- Assists the client family with formulation of short- and long-term goals.
- Directs individual patients in graded physical exercise programs according to tolerance level.
- Assists with Cardiac Rehab exercise groups.
- Collaborates with other departments and community organizations to optimize patient care.
- Consults with physicians as to patient progress.
- Understands MET levels and equivalent workloads, exercise prescription principles, level testing, incision care (Post CABG), recognition of and corrective actions taken upon occurrence of patient intolerance to exercise regime.
- Follows established standards and practices Phase II rehabilitation processes.
- Completes client assessment based on patient/family interview, examination, and review of clinical data; communicates client status and patient needs to physician and other team members.
- Recognizes emergencies and readily apparent changes in patient’s condition and seeks assistance as appropriate.
Who You Are:
- Graduate of an accredited school of nursing. Bachelor of Science in Nursing strongly preferred.
- Minimum of one (1) year of cardiac care experience
- New Hampshire Registered Nurse or Compact State Registered Nurse License. B
- CLS, ACLS required.
- Nurses are expected to pursue professional certification upon eligibility or within three years of entering the specialty practice.
- Ability to advance basic computer skills as the health system adds new systems or performs upgrades to existing systems.
- Work schedule may include 8-12 hours per day in addition to On-call status for call coverage when the department is closed. May be required to assume assignments in other areas of the cardiovascular service line to meet patient needs.
