Who We Are:

Elliot Health System’s Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) provide individual and group-based treatment of co-occurring substance use and mental health disorders. PHP and IOP are for individuals who need care that is more intensive than individual outpatient programs but not as intensive as residential treatment. Depending on their role, staff can provide case coordination, assist participants in accessing community-based services, provide medication management, evaluate participants for medical issues, and more. The teams coordinate closely with primary care providers on a biweekly basis to ensure that all aspects of patients’ wellbeing and health are addressed appropriately.

About the Job:

The Manchester Doorway Case Manager is responsible for providing casework services to clients enrolled in the Doorway program. The Case Manager Provides clinical case management interventions with patients and families that address personal and environmental issues to maximize emotional, social and physical wellbeing and the appropriate and efficient use of health care, substance use and mental health services across the continuum. Promotes efficient coordination of patient care through collaborative practice with the Program team. Coordinates patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care.

What You’ll Do:

  • Collaborates and participates with the multidisciplinary team in the ongoing clinical assessment of patient’s status/care management
  • Provides case management services in assisting the patient in obtaining housing, substance use treatment, employment, insurance benefits and any other needs
  • Monitors patient’s progress to ensure care is appropriate and timely
  • Coordinates transportation for clients to access SUD treatment services
  • Participates in regular staff meetings, staff trainings, and supervisory sessions, accepts the responsibility for aiding the development and team relationships as requested

Who You Are:

  • Bachelors Degree, Required
    • Major in criminal justice social services, behavioral health, psychology, or related field.
  • Twelve hours of continuing education and training in substance abuse and criminal justice issues, Required
  • Skilled in Microsoft office products, Required
