The inpatient pharmacy department is comprised of pharmacists and technicians working collaboratively to provide safe, timely and effective medication therapy to our patients. We operate a decentralized model for both medication distribution utilizing Omnicell automation, as well as clinically with pharmacists dedicated to the teams on our nursing units. About the Job
Under direction of the Pharmacist Supervisor Clinical Oncology, the Pharmacist Oncology will work in a multidisciplinary setting enhancing quality of medication dispensing, safety, and education at the Solinsky Center for Cancer Care.
What You’ll Do
- Responsible for reviewing medication orders and identifying and preventing drug related problems and adverse drug events.
- Responsible for preparation, compounding, and dispensing of chemotherapy, non[1]chemotherapy, and investigational drugs according to USP 797/800 regulations.
- Serve as a consultation resource for medical professionals answering inquiries on drug therapy and products in a decentralized role.
- Perform medication treatment teaches with patients to outline expectations of drug therapy including adverse effect management.
- Perform follow up calls for new chemotherapy starts, assessing compliance of home anti-emetic regimens and possible side effects.
- Optimize medication therapy in accordance with established collaborative practice agreements and recommendations based on current clinical guidelines.
- Build and maintain oncology and research regimen therapy plans in collaboration with EPICBeacon Team which conform to current oncology professional guidelines.
- Serve as PGY-1 oncology pharmacy preceptor within direction of the Residency Program Director Maintain all necessary preceptor requirements in accordance with American Society of Health-System Pharmacists (ASHP) accreditation. Coordinate other pharmacy learning experiences as necessary (ie. APPE, IPPE).
- Support the inpatient team regarding clinical oncology questions, along with preparation of medication therapy for inpatients and applicable outpatient infusion center patients (ex. Inpatient chemo, bladder instillation therapies).
What You’ll Need
Professional Pharmacy Bachelors Degree required. Pharm D Degree preferred.
Successful completion of PGY2 Residency specializing in oncology required. Board Certification in Oncology (BCOP) preferred.
One (1) year previous clinical pharmacy experience required.
Current New Hampshire license to practice Pharmacy or eligibility required.
Epic Beacon Oncology Module; Epic Willow Inpatient Module highly preferred.
