About the Job:

The Home Health and Hospice Liaison will build, establish and maintain strong referral relationships with physicians and physician practices, skilled nursing and assisted living facilities, hospital and discharge case managers, and the community ensuring all patients who would require and benefit from VNA services have access to those services. Promotes and educates about VNA programs and services.

What You’ll Do:

1. Increases volume of referrals by building long-term relationships and ensuring excellent customer service.

2. Makes regular site visits to facilitate referral coordination.

3. Identifies problems and adverse referral situations; and works collaboratively with referral sources and agency staff to correct same. Takes a lead role in any customer service and/or quality improvement efforts derived from customer satisfaction surveys or client concerns.

4. Maintains comprehensive working knowledge of coverage guidelines for VNA services and provides education to staff and physicians as needed.

5. Maintains comprehensive working knowledge of the VNA’s markets including major payer groups, key referral sources and competitors market positioning.

6. Implements marketing plan and proposes adjustments in strategy as market forces change

7. Collaborates with hospital staff & VNA staff to effectively transition patients to VNA Manchester, and monitor readmissions.

Who You Are:

.           Education: Graduate of an accredited school of nursing, education in Sales and Marketing or related field required.

Experience: Three (3) to five (5) years experience in sales and marketing, homecare, or related required.  

Certification/Licensure: Current Licensure in State of New Hampshire as a professional nurse preferred.

Software/Hardware: Familiar with MS Outlook, Work, Excel and/or Access. Familiar with setting up and maintaining a data base of contacts and activities.

Other: Valid Driver’s License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities required. Proof of auto mobile insurance required. Motor vehicle background report will be conducted.
