About the Job: Under the supervision of the Supervising/Referring Physical Therapist; implements specific treatment programs for individual patients specifically for pediatric, adolescent and geriatric patients according to the principles and practices of Physical therapy.
What You’ll Do:
- Administers Physical Therapy patient care programs, as planned with and directed by the referring/supervising Physical Therapist.
- Confers on a regular basis with the Supervising/Referring Physical Therapist in order to progress the patient’s rehabilitation program effectively.
- Documents according to VNA standards and processes.
- In collaboration with the referring Physical Therapist, develops and implements appropriate patient discharge plan including but not limited to: home exercise programs, equipment, patient/family education, referral to community services.
- Communicates regularly with physicians, nursing staff and interdisciplinary team members, individually, and through team meetings to ensure efficient coordination of the overall health care plan.
- Adheres to safety, organizational and ethical standards including VNA safety standards.
- Adheres to VNA policies, protocols and procedures and infection control standards.
- Ensures confidentiality of employee, legal, client/patient, budget and all company matters.
- Positively reflects the agency within VNA and the community.
Job Qualifications:
- Education: Graduate of an accredited Physical Therapy Assistant Program with an Associates Degree required.
- Experience: Minimum one (1) year of experience in health care setting performing similar work activities preferred.
- Certification/Licensure: Licensed as a Physical Therapist Assistant by the state of New Hampshire Office of Licenses Allied Health Professionals required. BLS, CPR required.
- Software/Hardware: Documentation on laptop computer, basic typing skills.
