About the Job:

The Training & Development Educator’s focus is to provide orientation, training, and overall mentorship to new clinicians within the agency in collaboration with the agency’s Education, Quality, and Management teams. Additionally, the Training & Development Educator will provide ongoing clinical guidance and education to tenured clinicians who demonstrate a need for additional clinical support and education; when the agency establishes training initiatives; or when requested by a director, manager, or the education coordinator. The Training & Development Educator demonstrates well-developed clinical competency as demonstrated by exemplary practice in the provision of direct patient care and guidance of clinical staff. The Training & Development Educator will demonstrate excellent documentation practices that align with Home Health and Hospice regulatory requirements and OASIS assessments. Works in tandem with the agency’s Education, Quality, and Management teams to provide support to all clinical team members, helping to ensure smooth agency operations to drive positive patient outcomes and support maximum reimbursement. The Training & Development Educator role is integral to clinical employee retention and job satisfaction.

What You’ll Do:

 Innovation & Inspiration

  • Performs safe, competent clinical care, effective communication, skilled decision-making, and collaboration among all clinical disciplines, administrative staff, and leadership.
  • Participates in professional enhancement activities. Examples include, but are not limited to: membership in VNA performance improvement committees/workgroups, health system initiatives, development of education materials, and activities to keep current with evidenced-based best practices.
  • Provides support to new clinicians and the clinical care team in developing integrated approaches to care.
  • Participates on the agency’s Performance Improvement Committee (PIC) and Infection Committee.

Collaboration & Continuity in Caring

  • Works alongside the Coordinator of Education Programs and Clinical Development to support the agency’s annual Education Plan, established Preceptor Plan and the agency’s orientation process.
  • Consults with and/or communicates questions and concerns to the Coordinator of Education Programs and Clinical Development, Clinical Manager, and/or Directors as appropriate.
  • Assists staff with the provision of care to assigned patients when needed.
  • Performs other duties as assigned.

Accountability

  • Demonstrates knowledge of, and compliance with the Centers for Medicare and Medicaid Services (CMS) Conditions of Participation (CoPs) for home health through proper clinical assessment and accurate documentation.
  • Demonstrates proficiency with OASIS assessments and documentation.
  • Effectively controls costs through innovative and economical utilization of agency resources, including clinical staff, materials, and equipment.
  • Assists in the mentorship of clinical staff as stewards of resources.

  • Applies well developed critical thinking skills in guiding and assisting clinicians to identify and address clinical concerns and promote optimal patient outcomes.
  • Serves as a role model; coaches and mentors all clinical staff, promotes teamwork. Models all instruction/education in alignment with clinical best practices and compliance with organizational policies and procedures.
  • Portrays self in a professional and respectful manner in all interactions between VNA employees, health system colleagues and referral sources/providers.
  • Serves as the designated resource for clinical staff on orientation and assigned new graduate nurses during their first year of employment, beyond their precepted orientation period.

Who You Are:

  • Requires the ability to plan and perform diversified duties requiring an extensive knowledge of a particular field and the use of a wide range of procedures. Demonstrates knowledge about specific subject (s) or client/patient population for which the incumbent is a resource.
  • Requires knowledge and skills in assessing needs, program planning and implementation and evaluating outcomes and responses in relation to the educational needs of the agency population.
  • Ability to transport self to attend meetings, events and visit other sites, hospitals, and organizations.
  • Ability to interact effectively with people of varied educational, socioeconomic, and ethnic backgrounds, skill levels and value systems.
  • Ability to obtain BCLS qualification and CPR-trainer certification during orientation period and maintain during employment.
  • Knowledge of educational principles in respect to the needs of adult learners and experience in application of those principles.

Job Qualifications

  • Education: Graduate of an accredited school of nursing or physical therapy (BSN required for nursing or must be actively enrolled in a program. Termination of a program without degree completion, will remove eligibility for the role).
  • Licensure/Certification: OASIS certification is required within 6 months of hire. License to practice as a Registered Nurse or Physical Therapist in the state of New Hampshire required. Life support certification (Heartsaver, BLS, ACLS, etc.) appropriate to patient population required.
  • Experience: Home Health experience is required. Minimum of two (2) years in an acute care setting preferred. Hospice experience preferred.
  • Knowledge: Must be at a clinical level that reflects proficiency and in-depth knowledge of home health clinical care and documentation practice(s).
  • Skills: Ability to advance basic computer skills as the health system adds new systems or performs upgrades to existing systems.
