About the Job:
The Behavioral Health Coordinator supports Foundation Counseling and Wellness by responding to all behavioral health requests for new patient appointments, coordinating provider schedules, and facilitating referrals across practices and community resources. This role serves as the first point of contact for patients seeking behavioral health services, conducting intake assessments, ensuring timely access to care, and assisting patients with insurance and financial information.


What You’ll Do:

  • Screen behavioral health referrals for appropriateness and coordinate timely scheduling with providers.
  • Conduct initial patient intakes, assess urgency of care needs, and refer to community resources or crisis services when appropriate.
  • Serve as a liaison for patients, providers, and managed care agents to ensure smooth care coordination.
  • Educate patients on services, policies, insurance benefits, and financial expectations.
  • Maintain up-to-date knowledge of behavioral health and social services available in Southern New Hampshire.
  • Support providers by gathering patient history and assisting with intake paperwork.


Who You Are:

  • Associate’s degree in Sociology, Psychology, Human Services, or related field required; Bachelor’s degree preferred.
  • Minimum 1 year of healthcare experience required; behavioral health or mental health setting preferred.
  • Skilled in compassionate patient communication, time management, and conflict resolution in stressful situations.
  • Ability to multitask, prioritize, and work independently as well as collaboratively in a fast-paced environment.
  • Knowledge of HIPAA regulations, confidentiality standards, and patient intake processes.
