Pediatrics Nurse Practitioner

We are seeking a compassionate and experienced Pediatric Nurse Practitioner (NP) to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Nashua, NH.

Responsibilities:

• Conduct Outpatient comprehensive assessments.
• Manage desktop for refills and triage questions by the nursing team.
• 36 patient contact/4 administrative hours per week, no nights or weekends
• Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient care.
• Document patient encounters and progress notes accurately and thoroughly.
• Stay up to date on the latest advancements in cardiac care through continuing education and professional development.

Qualifications:

• Master’s degree in nursing
• 2+ years of Family Medicine experience
• Strong clinical skills, includingassessment, diagnosis, and treatment planning.
• Excellent communication, collaboration, and interpersonal skills.
• Proficiency with EPIC EMR preferred
• Ability to work independently and as part of a team.
