About the Job:
The RN Resource Nurse, Advanced Primary Care is a dynamic clinical leader within the ambulatory setting, blending direct patient care, care coordination, team coaching, and population health management. This role supports high-quality, evidence-based care for patients attributed to primary care provider panels while fostering a culture of safety, collaboration, and continuous improvement.
What You’ll Do:
- Provide comprehensive RN care, including assessments, care planning, interventions, and evaluations across in‑person, telephonic, and telehealth encounters.
- Deliver chronic disease self‑management education for conditions such as diabetes, CHF, COPD, asthma, hypertension, and depression.
- Coordinate care across the continuum—hospital discharges, specialty referrals, community resources, and pre‑visit planning.
- Track and support complex patients to improve transitions of care and reduce readmissions.
- Serve as a liaison to internal teams and external community partners.
- Precept new clinical staff and support onboarding, coaching, and professional development.
- Promote evidence‑based practice, safety, and quality improvement initiatives.
- Monitor clinical quality metrics and recommend improvements.
- Facilitate care team huddles and collaborate with interdisciplinary partners.
- Maintain required licensure, certifications, and ongoing competencies.
- Support organizational initiatives including value‑based care, population health, and EMR optimization.
Who You Are:
- A licensed RN (NH or NLC) with at least 2 years of nursing experience, including 1 year in primary care.
- Skilled in chronic disease management, ambulatory care workflows, and EMR use (Epic preferred).
- A strong communicator with excellent organizational and time‑management abilities.
- A collaborative team player who can coach, mentor, and model best practices.
- Adaptable, resilient, and comfortable leading change in a fast‑paced clinical environment.
- Experienced in care coordination or clinical leadership (preferred but not required).
