About the Job:
The Value Based Care Resource Specialist is a highly motivated and detail-oriented team member focused on driving the success of our Value Based Care programs. This role plays a key part in improving patient outcomes, closing care gaps, addressing suspect conditions, mitigating negative attestations, and maximizing financial returns on quality incentive and risk capture programs. The specialist collaborates closely with primary care practices to ensure screenings are completed and care gaps are addressed, directly impacting both patient care and organizational performance.
What You’ll Do:
Proactively identify and close care gaps through collaboration with primary care teams and data analytics.
Identify and validate suspect conditions to ensure accurate diagnosis capture and maximize risk adjustment.
Review and manage negative attestations, resolve discrepancies, and educate providers on documentation best practices.
Develop and deliver educational sessions to providers and staff on optimizing value-based care programs.
Analyze clinical data, generate reports, and provide actionable insights to improve quality performance.
Collaborate with internal teams to develop strategies that enhance financial returns from value-based care initiatives.
Who You Are / Requirements:
Education: Bachelor’s degree in Nursing, Healthcare Administration, Public Health, or a related healthcare field (required).
Experience: 2–3 years in value-based care programs or healthcare performance.
Preferred: Certification in Risk Adjustment Coding (CRC), CPHQ, experience with healthcare payers, Epic EHR, data visualization tools, and quality reporting platforms.
Skills & Abilities:
Strong knowledge of risk capture and healthcare coding regulations.
Excellent communication and interpersonal skills.
Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Teams, Visio).
Strong analytical and strategic planning skills.
Ability to work independently, multitask, and manage competing priorities in a fast-paced environment.
